1095-C Tax Forms Available for Download at Benefitsolver

The 2024 1095-C Tax forms are ready for download in Benefitsolver now. Log into myDENSObenefits.com on a laptop or computer and find Personal Documents to view and download the form. 

 

Due to the Paperwork Burden Reduction Act signed in December, paper forms are no longer required unless requested. To request a paper form, please contact the Regional Benefits team:

Printed forms that are requested will be mailed to the home address on file in Oracle. It is not necessary to file the 1095-C with your 2024 income tax return, nor are you required to have the form prior to filing your return. Once downloaded, keep the 1095-C form with your other tax documents.

 

For those who participated in a Health Savings Account (HSA) in 2024, MyChoice Accounts through UMB Bank mailed a copy of the 1099-SA form to your home in January. This form is also available to download at myDENSObenefits.com. Click the MyChoice Account icon (piggy bank), select Statements, then Tax Statements to download or print. The 1099-SA form cannot be downloaded or printed from a mobile device – please use a computer.

2025 Benefits

New medical benefit vendor and pharmacy administrators are effective 1/1/2025.
DENSO welcomes United Healthcare, Optum Rx, and Surest in 2025! Welcome packets were sent to your home address in December. These included your new insurance ID card(s). If you did not receive your new ID cards, contact Member Services at 844-749-7834 and make sure your home address is up to date in Oracle.

Register for a HealthSafe ID account at myuhc.com, optumrx.com, or Surest.com (if applicable). Use the member ID on your ID card, or Social Security Number if prompted for your identification number. Your single HealthSafe ID gives you access to each of these sites. If you have issues registering for a HealthSafe ID, call the tech support number at 1-877-844-4999.

2024 Benefits

2024 benefits were effective January 1, 2024 through December 31, 2024. 2025 benefits began January 1, 2025.

DENSO associates are able to view Blue Cross Blue Shield of Michigan (BCBSM) claim history for two years after the BCBS plan closes after 12/31/2024.

  • View or download previous claims at bcbsm.com or the Blue Cross mobile app.
  • If you have questions after the BCBSM plan closes, call 888-605-2563.

Transparency in Coverage

Transparency in Coverage Final Rule Fact Sheet (CMS-9915-F)

According to the Federal Government's Transparency in Coverage rule, plans and issuers must make public machine-readable files (MRF) containing certain pricing information. Health plan price transparency helps consumers know the cost of a covered item or service before receiving care.  More requirements will go into effect starting on January 1, 2023 and January 1, 2024 which will provide additional access to pricing information and enhance consumers' ability to shop for the health care that best meets their needs.
